The following forms need to be completed by each student before commencing their work at Institute of High Voltage and High Current (IVAT):

1. Checklist of IVAT Laboratory Document (LF-01)
2. Occupational Safety Compliance Form (LF-02)
3. Application for Conducting Activities in IVAT Laboratory (LF-03)
4. Safe Operating Procedure Form
5. Job Hazard Analysis Form

For student who use chemical only :

1. Chemical Handover Form
2. Chemical Inventory
3. Chemical Register
4. SW Labeling Form ( If necessary/ waste)
5. Re – Labeling
