Introduction:
Welcome to the application form for Research Funding on Contract-Research & Contract-Research Double-Tax-Deduction (CR & CR-DTD). This application form consists of two sections, each requiring specific details and attachments.
SECTION A: PROJECT & PROJECT LEADER’S DETAILS
In this section, please provide information about your project and the project leader.
Attachment Requirements:
Proposal: Please attach a detailed proposal outlining your research project.
Turnitin Report: Submit a Turnitin report to ensure the originality of your proposal.
SECTION B: COMPANY/COLLABORATOR DETAILS
In this section, provide details about your company or collaborator involved in the project.
Attachment Requirements:
Term Sheet: (NOT REQUIRED for CR; COMPULSORY for CR with MoA & CR-DTD) – Attach the term sheet if applicable to your project.
Letter of Appointment: Submit the letter of appointment related to the project.
SSM Report: Attach the SSM (Suruhanjaya Syarikat Malaysia) report.
Company Profile: Provide a detailed company profile.
Insolvency Certificate: Submit the insolvency certificate if applicable.
Please ensure all sections are completed accurately and all required attachments are included. Incomplete applications may result in delays in processing or disqualification.
